ABA Case Manager/OPT Assistant

The purpose of this position is to serve as a front-line staff in the Outpatient Services Department. Primary populations served through this program include children and adolescents with serious emotional disturbances and/or intellectual or developmental disabilities, as well as adults with serious and persistent mental illnesses, co-occurring substance use disorders, and/or intellectual or developmental disabilities. Core responsibilities include the following:

Case Manager provides oversight and monitoring of children and adolescents receiving Applied Behavior Analysis (ABA) services through the Autism Benefit. Track and appropriately document Autism Benefit enrollments, authorizations, provider changes, and disenrollments. Complete Individual Plans of Service, Periodic Reviews, and at least monthly Progress Notes for an assigned caseload of Autism Benefit enrollees. Work closely with agency psychologist to ensure that Autism Benefit services are provided with fidelity to regional and state technical requirements. Collaborate with contract agencies providing ABA services to ensure appropriate case consultation and coordination.

Outpatient Assistant will provide administrative and clerical support to the OPT department with new and returning consumers. Completing ABD’s and discharge summaries. Contacting consumers to reschedule appointments or annual re-assessments. Tracking due dates for IPOS’s in OPT caseload and annual consents/coordination of care with signatures. Sending re-engagement letters, assisting with Medicaid enrollment, organizing and filing OPT paperwork

License/Certification:

Possession of a Bachelor Degree in Social Work or other closely related field such as human services. Two years of experience working in a community outpatient mental health setting is preferred.

CERTIFICATIONS, LICENSURE and REGISTRATIONS: Possession of a current Michigan license as appropriate to the field of training/study. Licensure as an LLBSW or LBSW is strongly preferred

Outpatient Therapist
Location: Bad Axe, Michigan- Full-time
We are seeking a dedicated and compassionate Outpatient Therapist to join our Outpatient Services Department. This role provides direct clinical services to children, adolescents, and adults living with serious emotional disturbances, intellectual or developmental disabilities, persistent mental illness, and co-occurring substance use disorders.
What You’ll Do
  • Provide individual and family therapy using evidence-based interventions
  • Facilitate at least one therapeutic or psycho-educational group (topics assigned based on agency need and staff expertise)
  • Deliver clinical support services including documentation, authorizations, and liaison work with schools, courts, physicians, and community agencies
  • Utilize person-centered and short-term treatment approaches
  • Maintain a productivity standard of 50% face-to-face consumer contact
  • Attend peer staffings, emergency services meetings, and regular supervision sessions
  • Participate in on-call rotation for after-hours emergency coverage and assist with daytime on-call as needed
  • Actively support culturally-informed, recovery-based practices, person-centered planning, and trauma-informed care
Qualifications
  • Education: Master’s degree in Social Work, Psychology, or a closely related human services field (required)
  • Experience: Minimum 2 years in a community outpatient mental health setting preferred
  • Licensure: Current Michigan license appropriate to field of study (LMSW and CAADC strongly preferred)

POSITION: Medical Assistant

GENERAL STATEMENT OF DUTIES:

The Medical Assistant/Licensed Nurse is responsible for completing consumer assessments, developing person-centered treatment plans, monitoring and managing assigned consumers’ health needs, and consulting with agency psychiatrists and clinicians. This role ensures safe, effective, and culturally competent care in alignment with recovery-based practices, trauma-informed approaches, and person-centered planning.

Key Responsibilities

· Conduct consumer assessments and support person-centered treatment planning.

· Administer medications (including injections) and monitor vital signs.

· Maintain accurate medication histories and ensure consents are up to date.

· Prepare records and lab work for psychiatric appointments.

· Collaborate with psychiatrists, clinicians, families, and community providers.

· Perform routine health screenings and recommend follow-up care.

· Assist with scheduling and tracking compliance programs (e.g., REMS).

MINIMUM EDUCATIONAL REQUIREMENTS:

· Must have graduated from an accredited Medical Assistant education program and be certified or registered as a Medical Assistant.

· Two years of experience as a Medical Assistant, experience in administering injections and Mental Health experience is preferred.

· BCLS (Basic Cardiac Life Support) certification is required.

· Valid Michigan Driver’s License is required.

GENERAL STATEMENT OF DUTIES:

As a member of the ACT Person-Centered Planning team, members provide services and supports to adults who are diagnosed with serious and persistent mental illness, which may include co-occurring serious mental illness with a developmental disability and/or a serious mental illness with co-occurring substance use disorder. These services and supports are based on the principles of recovery and self-determination. Additionally, the employee must exercise an appropriate level of independent decision-making and work effectively in a team-based environment. This position is responsible for different assessments to document level of need as well as where the person is at in recovery, development of a person-centered plan, monitoring, linking, and coordination of services provided at the agency, within the community or in the consumers’ home. Evening and weekend hours may be required.
