Persons with Intellectual & Developmental Disabilities

Most of our services are covered by Medicaid and Healthy MI insurances but if you have an emergency, anyone in Huron County may just walk in or call.  If you need assistance in arranging for services you may call our Access Center or speak to our customer service representative.

Crisis Intervention Services

Crisis Intervention Services are unscheduled activities conducted for the purpose of resolving a crisis situation requiring immediate attention.  Activities include crisis response, crisis line, assessment, referral, and direct therapy.

Inpatient Psychiatric Hospitalization

Inpatient psychiatric care may be used to treat a person with mental illness who requires care in a 24 hour medically structured and supervised facility.

Assessments – Health

Health assessment includes activities provided by a registered nurse, physician assistant, nurse practitioner, or dietitian to determine the beneficiary’s need for medical services.

Assessments – Psychiatric Evaluation

A comprehensive evaluation, performed face-to-face by a psychiatrist.

Assessments – Psychological Testing

Standardized psychological tests and measures rendered by full, limited-licensed, or temporary limited licensed psychologists.

Assessments – Other Assessments, Tests

Generally accepted professional assessments or tests, other than psychological tests, that are conducted by a mental health care professional within their scope of practice for the purposes of determining eligibility for specialty services and supports.

Treatment Planning

Treatment Planning – activities associated with the development and periodic review of the plan of service, including all aspects of the person-centered planning process, such as pre-meeting activities and external facilitation of person-centered planning.

Targeted Case Management

Targeted Case Management is a service that assists beneficiaries to design and implement strategies for obtaining services and supports that are goal-oriented and individualized. Services include assessment, planning, linkage, advocacy, coordination etc.

Fiscal Intermediary Services

Fiscal Intermediary Services are defined as services that assist  the adult beneficiary or a representative identified in the beneficiary’s individual plan of services, to meet the beneficiary’s goals of community participation and integration, independence or productivity while controlling his/her budget and choosing staff who will provide the services and supports identified in the IPOS and authorized by the PIHP.

Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is a set of intensive clinical, medical and psychosocial services provided by a mobile multi-disciplinary treatment team.

Crisis Residential Services – Adults

Crisis Residential Services is a short-term alternative to inpatient psychiatric services for beneficiaries experiencing an acute psychiatric crisis.  Services are provided in licensed and certified adult foster care settings.

Habilitation Supports Waiver – Community Living Supports

Habilitation Supports Waiver – Community Living Supports facilitate an individual’s independence and promote integration into the community.  The supports can be provided in the beneficiary’s residence (licensed facility, family home, own home or apartment).

Habilitation Supports Waiver – Enhanced Medical Equipment and Supplies

Habilitation Supports Waiver – Enhanced Medical Equipment and Supplies includes devices, supplies, controls, or appliances that are not available under regular Medicaid coverage or through other insurances.

Habilitation Supports Waiver – Enhanced Pharmacy

Habilitation Supports Waiver – Enhanced Pharmacy are physician-ordered, non-prescription “”medicine chest”” items as specified in the beneficiary’s support plan.

Habilitation Supports Waiver – Environmental Modifications

Habilitation Supports Waiver – Environmental Modifications are physical adaptations to the home and/or workplace required by the beneficiary’s support plan that are necessary to ensure the health, safety, and welfare of the beneficiary.

Habilitation Supports Waiver – Family Training

Habilitation Supports Waiver – Family Training is training and counseling services for the families of beneficiaries served on the waiver.  Training includes instructions about treatment regimens and use of equipment specified in the individual plan.

Habilitation Supports Waiver – Out-of-Home Non-Vocational

Habilitation Supports Waiver – Out-of-Home Non-Vocational is assistance with acquisition, retention, or improvement in self-help, socialization, adaptive skills, and the supports services, including transportation that takes place in a non-residential setting.

Habilitation Supports Waiver – Personal Emergency Response Systems

Habilitation Supports Waiver – Personal Emergency Response Systems are electronic devices that enable beneficiaries to secure help in the event of an emergency.

Habilitation Supports Waiver – Pre-Vocational Services

Habilitation Supports Waiver – Pre-Vocational Services are services aimed at preparing a beneficiary for employment, but that are non- job tasked-oriented skills that contribute to employability in paid employment in integrated, community setting.

Habilitation Supports Waiver – Private Duty Nursing

Habilitation Supports Waiver – Private Duty Nursing services consist of nursing procedures to meet an individual’s health needs that are directly related to his developmental disability.

Habilitation Supports Waiver – Respite

Habilitation Supports Waiver – Respite care may be provided in the waiver beneficiary’s home or place of residence, licensed foster care home, group home, licensed care facility, or home of a friend or relative.

Habilitation Supports Waiver – Supports Coordination

Habilitation Supports Waiver – Supports Coordination involves working with the waiver beneficiary, and others that are identified by the beneficiary in developing a written individual plan of services through the person-centered process.

Habilitation Supports Waiver – Supported Employment

Habilitation Supports Waiver – Supported Employment is the combination of ongoing support services and paid employment that enables the beneficiary to work in the community.

Assistive Technology 

Assistive Technology is an item or set of items that enable the individual to increase his ability to perform activities of daily living with a greater degree of independence than without them.

Community Living Supports

Community Living Supports are used to increase or maintain personal self-sufficiency, facilitating an individual’s achievement of community inclusion and participation, independence, or productivity.

Enhanced Pharmacy

Enhanced Pharmacy items are physician-ordered, nonprescription “medicine chest” items as specified in the individual’s plan of service.

Environmental Modifications

Environmental Modifications are physical adaptations to the beneficiary’s own home or apartment and/or work place.

Family Support and Training

Family Support and Training are services provided to family members of persons with serious mental illness, serious emotional disturbance or developmental disability for the purpose of assisting the family.

Housing Assistance

Housing Assistance is assistance with short-term, interim, or one-time-only expenses for beneficiaries transitioning from restrictive settings into more independent, integrated living arrangements.

Peer Delivered or Operated Support Services

Peer Delivered or Operated Support Services are programs that provide individuals with opportunities to learn and share coping skills and strategies, move into more active assistance and away from passive roles.

Prevention Direct Service Models

Prevention Direct Service Models are programs using individual, family and group interventions designed to reduce the incidence of behavioral, emotional or cognitive dysfunction, thus reducing the need.

Respite Care Services

Respite Care Services are services that are provided to assist in maintaining a goal of living in a natural community home by temporarily relieving the unpaid care giver.

Skill Building Assistance

Skill Building Assistance consists of activities that assist a beneficiary to increase his economic self-sufficiency and/or to engage in meaningful activities such as school, work, and/or volunteering.

Support and Service Coordination

Support and Service Coordination are functions performed by a supports coordinator, coordinator assistant, case manager assistant, supports and services broker, or otherwise designated representative of the PIHP.

Supported/Integrated Employment Services

Supported/Integrated Employment Services provide job development, initial and ongoing support services to assist beneficiaries to obtain and maintain paid employment that would otherwise be unachievable.

Behavior Treatment Plan Review

Any behavior treatment plan that proposes aversive, restrictive or intrusive techniques, or psycho-active medications for behavior control purposes [where the target behavior is not due to an active substantiated psychotic process], must be reviewed.

Health Services

Health Services are provided for improving the beneficiary’s overall health and ability to care for health-related needs.  This includes nursing services (on a per-visit basis, not on-going hourly care), dietary/nutritional services, maintenance, etc.

Medication Administration

Medication Administration is the process of giving a physician-prescribed oral medication, injection, intravenous (IV) or topical medication treatment to a beneficiary.  This shouldn’t be used as a separate coverage when other health services are utilized.

Medication Review

Medication Review is evaluating and monitoring medications, their effects, and the need for continuing or changing the medication regimen.  Medication review includes the administration of screening tools for the presence of extra pyramidal symptoms.

Nursing Facility Mental Health Monitoring

This service is the review of the beneficiary’s response to mental health treatment, including direct beneficiary contact and, consultation with nursing facility staff to determine whether recommendations from mental health assessments are carried out.

Occupational Therapy

Physician-prescribed activities provided by an occupational therapist currently registered by the State to determine the beneficiary’s need for services and to recommend a course of treatment.

Personal Care in Licensed Specialized Residential Setting

Personal care services are those services provided in accordance with an individual plan of service to assist a beneficiary in performing his own personal daily activities, provided only in a licensed foster care setting with a specialized certification.

Speech, Hearing and Language Therapy

Activities provided by a speech-language pathologist or licensed audiologist to determine the beneficiary’s need for services and to recommend a course of treatment.  Diagnostic, screening, preventive, or corrective services provided on an individual.

Therapy (Mental Health) Child and Adult, Individual, Family, Group (Prev. Clinic Services)

Treatment activities designed to reduce maladaptive behaviors, maximize behavioral self-control or restore normalized psychological functioning, reality orientation, re-motivation and emotional adjustment.  Evidence-based practices are included.
